Connecting and informing people about legal rights related to health care and end-of-life can improve access to care, and their wishes can be better recognized and respected.
Advance Care Planning helps people think about and share their wishes regarding future health and personal care. It can help people know their rights and tell others what would be important if they were too ill to communicate. SpeakUp is a cross-Canada initiative to improve awareness of Advance Care Planning. Although not created specifically for the unique needs of people experiencing homelessness, the SpeakUp workbook can be a useful tool to learn more about having difficult end-of-life conversations, including things to think about, terms and concepts to be aware of, and how to start planning.
